Underwriting exceptions written around a profession's pay pattern, and what each exception costs.
There is no federal or agency loan for doctors, teachers or police officers. What exists instead is a set of underwriting exceptions that lenders and, in some cases, government or state programmes write around a specific occupation's pay pattern: income from a signed contract that has not started yet, a different treatment of student debt, or a purchase incentive tied to public service. Understanding which exception a programme is actually granting — and what is being charged for it — is the whole of the analysis.

What it gives you
A signed contract can stand in for pay stubs
The most useful exception for a newly qualified professional. Where a programme permits it, an executed employment contract with a start date inside the programme's window is accepted as income, so a resident finishing training can buy before the first payslip exists.
Student debt is treated differently, and that is worth real money
Even in standard underwriting the treatment matters: Fannie Mae permits a documented income-driven repayment amount — including $0 — to be used in the debt ratio, while Freddie Mac requires 0.5% of the outstanding balance where the payment is zero. On a six-figure balance those two rules produce very different answers, and knowing which agency your lender is using is worth asking about.
Good Neighbor Next Door is a genuine federal programme
HUD sells eligible homes in designated revitalisation areas to law-enforcement officers, pre-K to 12 teachers, firefighters and emergency medical technicians at 50% off the list price, secured by a silent second mortgage that requires three years of owner-occupancy. It is not a lending gimmick; it is in the Code of Federal Regulations.
State housing agencies run occupation-linked programmes too
State housing finance agencies operate their own down payment assistance and rate programmes, some of them tied to public-service employment. In Wisconsin that agency is WHEDA. Eligibility, funding and terms are set by the agency, not by a lender, so start there rather than at a mortgage desk.

What this programme costs you
Every programme buys you something by charging you something else. This is the half a brochure leaves out, and it is the half that decides whether the programme is right for you.

An exception is priced, even when it is not itemised
A programme that waives mortgage insurance above 80% loan-to-value has not made the risk disappear. It has moved it into the rate, or into a narrower investor list, or into a prepayment structure. That may still be the right trade — but it is a trade, and comparing the programme against a conventional loan with PMI over the years you will actually hold it is the only way to know which is cheaper. Frequently the answer is the ordinary loan.

Leaving student debt out of the ratio does not leave it out of your life
This is the specific trap in physician programmes. Excluding a deferred or income-driven student loan payment from the debt-to-income calculation qualifies you for a larger mortgage. The loan itself continues, usually accruing, and the payment usually rises as your income does. The underwriting has been relaxed; your budget has not. Model the mortgage payment alongside the student loan payment you will actually be making in three years, not the one on the credit report today.

Eligibility is narrow, verified, and can lapse before closing
Start-date windows, licence checks, degree verification, employer type, and a contract that must remain in force. A delayed start date, a change of employer between approval and closing, or a licence that has not issued yet can void the approval days before you are due to close — with a purchase contract already signed.

Good Neighbor Next Door has real strings
The 50% discount is secured by a second mortgage for the discounted half, and you must live in the property as your sole residence for 36 months. Homes are sold as-is, availability is limited to designated areas and short listing windows, and ties are settled by lottery. It is an excellent programme for the right buyer and a poor fit for anyone who might move.

These are rules, not prices. This site publishes no rates, so what follows is the part of the programme that is written down somewhere a third party can check.

Who it fits
The usual guidelines. Every one of them is a starting point that an underwriter reads against the whole file, so treat a line you miss as a conversation rather than a closed door.
- Membership of the specific occupation the programme names, verified — not merely a related field
- For contract-start income: an executed employment contract with a start date within the programme's window
- For Good Neighbor Next Door: employment as a law-enforcement officer, pre-K to 12 teacher, firefighter or EMT, and a HUD-listed home in a revitalisation area
- For state agency programmes: the income, price and area limits the agency sets
- Ordinary credit, asset and property standards of the underlying loan
- Some programmes require the property to be your principal residence and hold you to it

Establish which exception you actually need
Income before a start date? Student debt treatment? Down payment help? Each has a different answer, and some of them are available inside an ordinary conventional loan without a special programme at all.
Check the state housing finance agency first
Agency programmes are usually cheaper than lender-branded ones and their rules are published. If one fits, it is the better starting point.
Price the programme against an ordinary loan
Run the branded programme and a standard conventional loan with mortgage insurance over the number of years you expect to hold the loan. Compare the totals, not the features.
Verify the eligibility documents early
Contract, licence, degree, employer letter. These are the items that fail late, and they are the ones you can chase now.
Underwriting and closing
Expect the eligibility to be re-verified close to closing. Any change in employer or start date has to be disclosed immediately, not explained afterwards.

Questions this raises
Is there really a special mortgage for my profession?
There is no agency programme for any occupation. What exists is private investor products built around an occupation's pay pattern, plus two genuine public routes: HUD's Good Neighbor Next Door for law enforcement, teachers, firefighters and EMTs, and the programmes each state housing finance agency runs. Everything else is an underwriting exception with a name attached, which does not make it bad — it makes it something to price rather than something to accept.
Physician loans skip mortgage insurance. Is that free?
No. The risk of a low-down-payment loan does not vanish because the borrower is a doctor; it is recovered somewhere else, most often in the rate and sometimes in a narrower set of terms. Whether that is cheaper than a conventional loan with PMI depends on your credit score, the down payment and how long you keep the loan — and because conventional PMI cancels at 80%, the comparison often reverses a few years in. Run both over your real horizon.
How is student loan debt counted if my payment is $0?
It depends which agency's rules your lender is applying. Fannie Mae permits a documented income-driven payment to be used even when it is $0. Freddie Mac requires 0.5% of the outstanding balance to be used instead. On a large balance those produce very different qualifying amounts, so it is a reasonable question to ask a lender directly — and the answer may point you to a different lender rather than a different programme.
What is the catch with Good Neighbor Next Door?
The discount is real and so are the conditions. Half the list price is secured by a silent second mortgage that is forgiven only after you have lived in the home as your sole residence for 36 months. Properties are HUD-owned, sold as-is, confined to designated revitalisation areas, listed for a short window, and allocated by lottery when more than one eligible buyer bids. It is an excellent programme for someone who is certain about staying and a poor one for anyone who might move.
